The form used in my parish at Barnet for over 20 years is exactly that given by Gabriel (give or take the odd rest), but I cannot remember how I originally acquired it.
At the time, not knowing of any version of the 'For the kingdom' acclamation, I invented my own. More recently, my PP sensibly suggested that the embolism should be sung to bridge the gap between the 'Our Father' and the acclamation (see Celebrating the Mass 201), so I have set that too.
Here is the Barnet Rite version:

Excellent! I think the Council of Trent gave the seal of approval to any liturgical usage that had been around for at least two hundred years. How long have you been in charge in your parish, John?
